Everyone is entitled to care following discharge from the hospital to help them transition back into their lives.
If returning home isn’t possible due to the need for nursing assistance or 24-hour care, it’s vital to know the next steps.
Care assessment
If someone is likely to have long-term health and care needs, a care assessment should be completed as soon as possible.
This assessment determines the type of care needed, who will provide it, and how it will be funded.
Registered Nursing Care Contribution
If care from a registered nurse or doctor is needed, you could be entitled to the Registered Nursing Care Contribution. This means and element of care will be funded by the NHS.
Local authority support
If the above options do not apply, the Local Authority will assess your financial means to determine your contribution.
The current threshold is £23,250. If you have savings beyond this amount and none of the above applies, you will be considered self-funded and responsible for all care costs. Any income you receive by way of pensions will be taken into account.
Property and care costs
A common misconception is that your property must be sold to pay for care. This is not always the case – for instance, if your spouse or someone is dependent lives in the property, it is disregarded in the assessment.
You can also request a deferred payment agreement, meaning the property is sold only after your death, and care costs are recovered from the sale proceeds.
Asset transfers
Transferring assets to children, either outright or in a Trust, to avoid care costs may be considered a deliberate deprivation of assets.
These transfers will still be counted towards financial assessments. Therefore, it is crucial to obtain expert advice before pursuing these options.
